Prison Chaplain or Resettlement Officer
If you know of an individual who may be interested in joining the Bridge Programme, please do get in touch.
If you know of someone who would like support on release through the Bridge Programme, please ask them to complete an Application form. You will need to complete the short Chaplain's Assessment and send both forms to us. We will be in contact with Probation for risk assessment information.
Application form - this must be completed and signed by the individual in order to progress their application and obtain permission to request information from their Chaplain and Resettlement or Probation Officers.
Chaplain's Assessment - this provides valuable insight into the character of the person and the type of support that may be required. A signed application form gives us permission to request this information.
